You will be required to fill out minimal information for a new application. Renewals are stored in the system and all that is required is your AMA number. You will then be prompted to pay with PayPal. Simply click on the link, complete the form, print, and you&#8217;re ready to send us your form! If you email your application, your payment must be received with in one week.
